For us to provide you with the best possible treatment and care, please complete the following Patient Forms prior to your first appointment. You have the option of filling it out electronically and then printing OR printing and completing by hand.  If you are unable to do either prior to your first visit, please arrive 20 minutes before your appointed time in order to complete them in our office.  This will enable us to seat you in a timely manner.
